Prepare For Your First Appointment

Our pain management doctors in Scottsdale are conveniently located right off Loop 101 at 91st Street. In order to provide convenient appointment times for all of our patients, we are open from 7:00 a.m. to 5:00 p.m. Monday through Friday. For clinic appointments, we kindly request that patients arrive 15 minutes before their scheduled appointment time.

If you are being seen by one of our providers for the first time, we ask that you arrive 30 minutes early. This will give you adequate time to complete our new patient intake paperwork. If you prefer, you can download, print, and complete these patient forms prior to your visit, using this link

We accept most insurance plans, including government-sponsored plans and private commercial insurances. Our complete list of accepted insurances may change, so be sure to call our patient concierge team with any questions. They will explain your insurance options, which may include out-of-network benefits. If you are out-of-network, we can help offer options that makes your care more affordable.
